Breakthru Community Transition Program

Breakthru is a comprehensive and effective approach to enabling people with disability or those living with complex mental health to successfully transition back to society from prison or a secure mental health setting, achieving long-term stability in all areas of their life and within their local community.

Stepped Care Program

Breakthru offers a stepped care approach combining 3 unique and targeted components, each of which can be commissioned as individual programs:

1. 90-Day Resettlement Program
2. Specialist Supported Independent Living
3. Specialist In-home & Community Outreach

Specialist Supported Independent Living

Specialist supported independent living service offers high intensity, flexible, and person centred 24/7 support on a 1:1, 1:2 or 1:3 staffing ratio, providing individuals with a safe, secure, and stable place to live, reducing their exposure to dangerous and unstable living conditions and supoprt settings, and promoting a sense of belonging and community.

Specialist In-Home & Community Outreach

Specialist drop in and community outreach services are for individuals who present a low risk of reoffending or readmission to hospital as a result of unmet need. Provided on a flexible 1:1 basis, these supports play a crucial role in preventing relapse or recidivism by providing regular assistance and guidance in familiar environments, such as the person’s own home or local community,

90 Day Resettlement Program

Supported transition for a period of up to 90 days in specialist medium term accommodation, providing a structured and supportive environment that promotes safety and stability, assessment, planning, treatment continuity, crisis and risk management, skill development, well-being, and a staged reintegration back to community.

Eligibility Criteria

  • Resident or able to reside in SE Queensland

  • Aged 18+ years

  • Non-violent criminal background

  • Diagnosed disability or

  • Diagnosed mental health condition

  • History of reoffending

  • Leaving Prison

  • Leaving secure hospital setting

  • NDIS participant

  • High intensity support needs

Exclusion Criteria

Individuals who meet all other criteria but who:

  • have committed serious offences such as murder, homicide or manslaughter and sex offenders who have not undergone rehabilitation.

  • require low or semi secure support setting

Referral Process

  1. Confirm eligibility against Breakthru's eligibility criteria.

  2. Complete Breakthru's referral form, ensuring that all sections are completed fully.

  3. Gather all supporting information including an up to date risk assessment and case summary.

  4. Submit the referral and all supporting information to info@afnc.com.au

  5. Failure to complete the referral form in full may result in delays in processing and responding to your enquiry

Assessment Process

  1. All referrals will be reviewed within 72 hours of receipt by AFNC's Nurse Practitioner.

  2. Referrers will be notified within 5 working days of the outcome of their referral.

  3. An in-person assessment undertaken by AFNC's Nurse Practitioner will be required for all eligible referrals.

  4. Upon acceptance of a referral, a dedicated AFNC transition coordinator will work with identified stakeholder sto ensure a smooth transfer to the Breakthru program.
